Services Available for Online Booking (all appointments scheduled in the clinic, call if you require a virtual appointment):
Appointments for workplace injuries, bundled care, and motor vehicle collisions cannot be booked online, please call the clinic.
Initial Assessment: new injuries that have not been assessed at our clinic or that we have not treated in the past 3 months (please call if you are unsure).
Concussion Assessment: assessment of concussion and related symptoms. This assessment is available with Derrick Owen PT and Darcy Trefiak PT.
Vestibular Assessment: assessment of vestibular or dizziness symptoms including BPPV and PPPD. This assessment is available with Derrick Owen PT.
Pelvic Health Physiotherapy Initial Assessment: assessment of pelvic floor dysfunction. This assessment is available with Amishi Shah PT.
Follow Up: repeat appointment for symptoms that have been assessed within the last 3 months.
Pelvic Health Physiotherapy Follow Up: repeat appointment for symptoms that have been assessed within the last 3 months. Available with Amishi Shah PT.

This site can be used to see availability for WSIB (workplace injuries), OHIP (physiotherapy services covered by OHIP - these are only available in certain circumstances) and Bundled Care (knee, hip and shoulder replacements) appointments. Please call the clinic to book.
Cancellations and No Shows:
Your appointment time is reserved especially for you.
If you are unable to keep your appointment, please provide at least 24 hours notice. Within 24 hours, you must contact the clinic to cancel.
Cancellations within 24 hours or failure to attend your appointment will result in a charge of $50

Derrick is a Registered Physiotherapist at Physiotherapy Alliance’s New Hamburg location. He began his studies by completing his Bachelor of Science Human Kinetics degree at the University of Guelph in 2013, then went on to complete his Master of Physical Therapy degree at the University of Saskatchewan in 2016. Derrick has a special interest in the treatment of a variety of orthopedic injuries and conditions, vestibular rehabilitation and vertigo, and sports injuries prevention and rehabilitation. He has completed his Advanced Manual and Manipulative Physical Therapy Diploma and is a Fellow of the Canadian Academy of Manipulative Physiotherapy. He has also completed post-graduate training in Acupuncture and Vestibular Rehabilitation. Derrick looks forward to continuing to pursue further training in these areas of physiotherapy to further expand his repertoire in the near future.
In his spare time, Derrick enjoys spending time with his wife and their daughter. He also enjoys all things sports, especially hockey and golf.

Amishi Shah
Amishi is a Physiotherapist working at the New Hamburg location. Amishi grew up in India and graduated with a Bachelor of Physiotherapy from Garden City University in 2022. She later moved to Canada becoming a registered physiotherapist and pursuing further education.
She believes in providing evidence-based, patient-centered care, where treatment is tailored to each individual’s goals and needs. Amishi is passionate about helping clients restore movement and function through therapeutic exercise, manual therapy, and education. She has completed post-graduate education in pelvic floor physiotherapy, and looks forward to completing additional studies in vestibular rehabilitation, acupuncture, and manual therapy in the near future.

Abbey is a Physiotherapist working at the Stratford location. In 2020, she completed her Honours Bachelor of Kinesiology at Brock University, where she then continued on to complete her Master of Professional Kinesiology in 2021. Abbey completed her Doctor of Physiotherapy at Bond University in Australia in June of 2024. Abbey’s areas of interest include orthopedic injuries and conditions, as well as sport-related injury, prevention, and return to sport. Abbey is eager to participate in post-graduate education in acupuncture and manual therapy in the near future.
